Transition
Transition from childhood to adulthood is an important phase in every child’s life and is often marked by significant periods of change and progress. Changes can include vocational aspirations, living arrangements, educational transition, financial planning, and dynamic shifts in social and developmental needs.
For many children with special health care needs, preparation for this event (e.g., planning and outlining goals and supports) must be considered extensively during the teenage years. At About Families we believe that each transition plan should be client driven and developed to surround young adults with a supportive, trusted network of family, friends, and providers.
Planning
During our initial visit, we ask families and children what their long term goals are, regardless of their current age. It is never too early to begin planning and About Families will work with you on outreaching services and developing life goals that can be addressed at each stage of development in order to insure that children and families are able to maximize their potential.
Our teams of qualified staff have extensive experience supporting their own children through adulthood and have had the opportunity to work closely with schools and adult service providers to help other children transition into adult services.
