SERVICES
About Families CEDARR Center serves children statewide in locations accessible to families, including the home and community sites (transportation for the family can be arranged). With a parent's or guardian's consent, any individual or organization can refer a child for services, including a primary care physician, the school department or early intervention, specialists, private providers, and advocates.
When a family seeks services through a CEDARR Family Center, there are several basic services that may be provided:
Basis Services & Supports: Often families just need information to help them proceed with getting care for their child. About Families CEDARR Center responds to this type of initial family contact with provision of information about special needs resources, systems mapping and navigation, and peer family support and guidance.
Initial Family Assessment: This assessment is conducted to identify a child’s and family’s strengths and needs in preparation for planning services and treatment.
Specialty Clinical Evaluation & Treatment Consultation: An array of specialized evaluations may be conducted to inform the family of their child’s special health care needs, and help identify service and treatment to meet those needs. Treatment consultation with the child’s primary care physician(s) may also by used for additional input.
Family Care Planning: The parent-professional team meets with potential service providers to develop a comprehensive plan that will build on the strengths of the family and integrate the assessment information. The plan targets goals, identifies needed supports, and delineates responsibilities of community providers.
Family Care Coordination: The goal of care coordination is to help the family link to services, to gradually gain independence in coordinating the care of their child, and to monitor and evaluate the goals in the Family Care Plan.
Crisis Intervention: About Families CEDARR Center responds to the emergency needs of children and families 24 hours/day, 7 days/week through its own clinical triage team and/or connection to existing emergency services. The Center also follows-up with the family to plan future care needs.
Enhanced Services: These services include a limited amount of direct support to the family to help in prioritizing needs, demonstrating interventions, and strengthening family skills.
