Care Coordination & Review

After the Family Care Plan has been reviewed with the family and child, About Families will work with the family on implementing the Family Care Plan. While About Families works on empowering families to be as active as possible in their children’s care, we work hand in hand with families on accessing supports and overcoming barriers to treatment. Some areas in which we may offer support are:

  • Transitioning out of Early Intervention (EI)
  • Referrals to mental/behavioral health providers
  • Outreach to Speech, OT, PT, Sensory Evaluations
  • Heating, Housing, Resource Assistance
  • Working with schools to help families understand the IEP process
  • Transitioning children to Adult Services
  • Obtaining Durable Medical Equipment (DME)
  • Accessing and coordinating CEDARR Direct Services
  • Providing Therapeutic Counseling to families
  • Consulting with Managed Care Plans to secure benefits

Throughout this process, we encourage families to be the lead in their care plans and will tailor our services to meet the varying needs and capabilities of our children and families.

Family Care Plan Review

While we are in regular contact with families, in order to be certain that services remain appropriate, we will conduct a review of each of our client’s Family Care Plans at least once every 12 months. With the family’s permission, we will include current treatment providers in this review process to encourage continuity of care across all services. During this review, goals and services will be amended as needed, with follow up care coordination taking place with current and new treatment providers. We will give the family a chance to discuss progress on goals as outlined in the Family Care Plan as well as the chance to add any new goals to reflect the child’s changing needs and treatment gains.

By continually reviewing the child’s and family’s progress, we can ensure that the Family Care Plan always reflects the most appropriate needs of the child and that everyone involved is in agreement with the actions needed to be taken to help support the child and family.
