Initial Family Intake and Needs Determination

During this home based assessment the goal is to begin to determine the level of CEDARR services appropriate to the child and family’s needs. About Families staff will attempt to include all immediate family members and any other individuals that the family identifies as being helpful and understanding of the child’s needs in these activities. In completing the Initial Family Intake and Needs Determination, staff may work with the family to obtain information about any of the following:

  • Assessment of urgency
  • Developmental and diagnostic history
    • Child and Family Background and Functioning
    • Medical History
    • Psychiatric History
    • Educational History
    • Social and Recreational History
  • Current and past interactions with other providers, including any relevant documentation to avoid duplication of effort
  • Participation with other programs (e.g., Special Education, Early Intervention, DCYF programs, etc.)
  • Care Coordination through RIte Care Health Plans, Commercial Insurers, and other managed care plans
  • Family strengths, needs, and supports
  • Social, community, and spiritual involvements and supports
  • Knowledge of or linkage with advocacy groups and/or professional associations
  • Current insurance status
  • Cultural or language needs that may impact services
