Initial Family Contact
Families or providers interested in About Families CEDARR services should contact our Pawtucket office. During this call families and professionals will be asked to provide basic identifying and insurance information.
In order to insure the quickest access to services, please be ready to provide:
- Name of the child for whom services are being sought
- Parent/Guardian of child for whom services are being sought
- Social security number of child
- Address of child
- Relationship of caller to child
- Insurance Carrier and Insurance Identification number of child
- Reason for referral/contact
- Signed release of information form if caller is someone other than the child’s guardian
Once this information is obtained, the child’s information will be given to a Clinical team at About Families who will contact the family within a week to schedule a date for the in home evaluation.
Unless a different date is requested by a family, in home evaluations generally occur as soon as 5-10 days from the date of the initial phone call to About Families.
