Family Care Plan
After the IFIND meeting and once an appropriate diagnosis has been determined, families and staff will work to develop a Family Care Plan. In developing a child’s Family Care Plan, we will incorporate all of the information obtained at the IFIND meeting, as well as information secured from past evaluations, service providers, and clinical observations. This document will serve as a roadmap to our services. It will include information such as:
- Current Demographic Information
- Mental Health/Behavioral Health History
- Medical History
- Developmental History
- Family Dynamics
- Educational History
- Current Providers with whom the family is working
- Strengths and Issues
Strengths and Issues
The Strengths and Issues section of the Family Care Plan will work to capitalize on the family’s and child’s individual strengths. By recognizing the family’s strengths in the Family Care Plan we are able to help a family address the specific needs of the child in a way that is familiar to and manageable by all those involved. This section will outline steps that the family and staff will take in outreaching providers; consulting with community and school supports; and making personal, family changes in order to meet the needs of the child.
