THE FAMILY’S EXPERIENCE
Welcome Families!
We understand how difficult it is to find the right combination of health and social services that will help you manage everyday activities and improve your quality of life. Here’s what will happen when you call About Families CEDARR Center for help:
First, one of our staff representatives will ask you several questions on the phone to help us better understand your child’s needs. We will schedule a first appointment, typically at your home or in our offices.
Your initial meeting will usually be with a Family Service Coordinator who is also a family member of a child with special needs. A Clinician will team with the Family Service Coordinator to meet with you 2-3 times to gather information and observe your child to better understand his/her strengths and needs.
In addition, we may ask that a specialist evaluate your child in areas such as speech/language, neurodevelopmental assessment, occupational therapy, clinical psychology, psychiatry, etc. We will facilitate setting up these appointments and providing information to aid in the evaluations.
When the assessment process is completed, we will meet again with you and representatives from other agencies who provide services (schools, therapists, doctors, etc.) to develop a Family Care Plan that will guide us in connecting you and your child to the right services.
Our staff will then help you get the services started and make sure your child’s care is coordinated. We will communicate regularly with you and your providers to monitor your child’s progress.
Every six months (or more often if needed), we will review the Family Care Plan with you and all providers. CEDARR services are maintained as long as they are needed to support continued progress of your child. There is no obligation to continue receiving services.
